I don't know for the rest of Europe, but in Spain we have:
- One (or more) first name. In my case, José Carlos is my name. My friends just call me José or José Carlos, both are fine. There're many names made by José something in Spain: José Antonio, José Manuel, José Luis, José Ramón, José Viecente... Practically we can use José plus any other name in Spain. In girls, the same applies to María (José and María are the spanish for Joseph and Mary, the parent of Jesuschrist). We have María Dolores, María Isabel, María Clara... Even the combinations José María (man) and María José (woman) are habitual.
- Then we have the first part of the surname of the father: My father is José Ginés Martínez Zamora, so I get the Martínez.
- Finally we have the first part of the surname of the mother: Mine is Encarna Galán Ayuso, so I get Galán.
We don't have such a concept of middle name here. The first name is the first name, no matter how many words it has. Some people are just called Juan and some other I heard was called "Felipe Juan Froilán de Todos los Santos". Long, but first name anyway
